INSTRUCTIONS
Please read the instructions carefully and upload your documents accordingly.
You do not need to upload all documents at once. You may upload in as many sessions as you would like. For example, you can upload just your photo for now, if you don't have a scan of your passport with you. Later you can upload your passport. Just make sure to enter your full name again at the top.

press submit. There will be a message that your documents were uploaded.
FILE DETAILS
- Personal Photo (jpg): Please save the file with your Full Name and your Passport Number. For example: Martha Pollack 585230985.jpg
- Scanned Copy of Passport (.pdf): Please save the file with your Full Name and the word Passport. For example: Martha Pollack Passport.pdf
- Medical Form (.pdf): Please save the file with your Full Name and the word Resume. For example: Martha Pollack Medical Form.pdf
- Vaccination Certificate (.pdf): Please save the file with your Full Name and the word Vaccination. For example: Martha Pollack Vaccination.pdf
